Some good pointers in terms of institionalized relationships, brand and client acquisition are good points, but 99% of independent practices are far from being to worry about doing anything about this.

Mark Hurley was very informative, if a bit brusque; he doesn't beat around the bush and has strong opinion in this area. But, with his research, he may be qualified to be that way.
